If you aren't 18 or older, you cannot grant permission to yourself to act as your own guardian. It kind of defeats the purpose of the consent form. It's kind of like asking yourself if you can borrow your friend's car. Sure, it's okay with you, but it might not be okay with the person in charge (namely the car's owner).
What you cando is have your parent or guardian grant permission for you to act as your own guardian, and then they sign the release form. If the parent is granting the permission, then you're okay. But you can't grant yourself permission.
