Signed form for minors?

I know I had to bring in a form with me at Sakuracon, since I wasn't with my parent, although I was with an adult friend.
Will I need to bring a signed form from my parent when I pick up my badge on friday?
(I'm nearly 17, so I'm pretty sure the under 12 stuff is beyond me.)
Need to get all these random details smoothed out.

Most people sent that form in when they registered. I suggest bringing the form with you on Friday night just in case.
