Ah, there is the form.  It's just a bit hard to see.  It's on the left under the blue sidebar.

I was at the Kitty table too... Part of the group that had to leave early. That was a lot of fun. My cousin had to miss it so that another friend wouldn't be left all by herself, so this list is epically helpful for us.
